At St. Jude’s, we believe every person bears the image of God and brings gifts that strengthen our common life. As we continue growing into a community where all belong, we are taking time to listen more deeply to the experiences of parishioners who live with disabilities, chronic illness, neurodivergence, sensory differences, or mobility challenges, as well as those who care for or accompany someone who does.

This survey is part of a diocesan pilot project that I have been asked to lead, exploring how parishes across the Diocese of Niagara can better understand and respond to the lived experience of disability in their communities. St. Jude’s is the first parish participating in this trial, and your insight will help shape not only our own next steps but also the wider diocesan learning that will follow.

The survey will take approximately twenty minutes to complete. It will be helpful if you answer the questions as fully as you can, however, you may answer as much or as little as you wish. All responses are confidential unless you choose to identify yourself. What matters most is we hear your perspective—your lived experience, your hopes, and the ways you imagine St. Jude’s becoming an even more compassionate and accessible place of belonging for all.

If you have difficulty with the online survey format, a paper version is available from the church office. You may also request assistance completing the survey by contact staff at the church office.

Thank you for taking the time to share. Your voice will guide both our parish and the diocese as we seek to embody Christ’s welcome with greater attentiveness, humility, and joy.
